There are two distinct types of insurance - one for locations and one for equipment rental from a third party vendor.

Location Insurance

Location Use Agreement

The Location Use Agreement form is required when filming on a location other than DePaul property. The agreement states that the project is covered by DePaul insurance. The form must be signed by the property owner and an authorized faculty or staff member representing DePaul University. Copies of the signed forms must be retained by the producer of the film for two years after completion of the project.

The Production Office staff will assist with obtaining a signature from an authorized staff member representing DePaul. Please obtain the property owner’s signature prior to bringing the form to the Production Office.

Location Certificate of Insurance

If the property owner is requesting a Certificate of Insurance (COI), in addition to the Location Use Agreement, complete the following steps:

  1. Complete the Insurance Request Form through the Office of Risk Management.
  2. Forward the online request confirmation email from the Office of Risk Management along with the signed Location Use Agreement to production@cdm.depaul.edu.
  3. Once approved, you will be sent a copy of the Certificate of Insurance.
  4. A copy must be retained by the producer of the film for two years after completion of the project.
  5. There is no "blanket" insurance for multiple location shoots. This procedure must be repeated for each location.
  6. If a location owner or representative is requiring more than proof of insurance, requests should be directed to a Production Office staff member.
  7. All requests must be submitted three weeks prior to the date the Certificate of Insurance is required.

If you are applying for a film permit from the City of Chicago or the Chicago Parks District, you will be required to obtain a COI. The Production Office has pre-made COIs for these two institutions available upon request.

Equipment Rental Contracts from a Third Party Vendor

This Rental Equipment Request form is used when renting equipment from an outside vendor. Only an authorized staff member representing DePaul University may sign equipment rental contracts. The Production Office staff will assist with obtaining a signature from an authorized staff member representing DePaul. The following steps must be completed:

  1. Complete the Rental Equipment Request form.
  2. Obtain a quote with insurance values of the equipment to be rented, length of rental, and vendor contract and/or insurance requirements.
  3. Submit completed form and quote with script pages to be filmed to the Production Office for approval.
  4. Once approved, complete the online insurance request form through the Office of Risk Management.
  5. Forward the online request confirmation email from the Office of Risk Management to production@cdm.depaul.edu.
  6. Once approved, you will be sent a copy of the Certificate of Insurance. A copy must be retained by the producer of the film for two years after completion of the project.
  7. There is no "blanket" insurance for multiple third party vendors. This procedure must be repeated for each vendor.
  8. All requests must be submitted to the Office of Risk Management three weeks prior to the date the Certificate of Insurance is required.

Working with Independent Contractors

If you need to hire, whether paid or unpaid, a non-student technician or operator, you may wish to enter into a contractual agreement for the hired services. If that is the case, the Production Office can provide you with a standard contract. The Production Office can also provide a standard contract for the use of another person’s intellectual property in your project. Please contact us at production@cdm.depaul.edu for more information.
