Vendor and Volunteer COVID-19 Vaccine Compliance Certification

Starting on September 20th, 2021, vendors and individuals providing services or volunteering at the university, both on campus and at affiliated off-campus locations, are required to be vaccinated for COVID-19, or have a qualifying religious or medical exemption coupled with a weekly testing requirement. This group includes guest lecturers, models in art classes, and any other service providers.

Vendors and others providing services to the university, including volunteer services, must certify that they are in compliance with the policy by completing an annual certification form (below).

A CDM faculty or staff member who invites a vendor or guest on campus and at an affiliated off-campus location is responsible for ensuring that the vendor/guest has received and agreed to the Vendor and Volunteer COVID-19 Vaccine Compliance Certification policy by completing the relevant electronic form here:

No vendor or individual should be working or volunteering on campus, or at affiliated off-campus locations, after Sept. 20, 2021, without having a signed compliance form on file.
