Below are common forms current students might need over the course of their academic career.
Actor and Crew Release Form (NON-Student over 18)
This form is required for use by students hiring actors or crew members, over 18, who are not DePaul students when they participate in a DePaul film. The producer and/or director of the film is responsible for obtaining the signed form and retaining it for one year after the completion of the film project.
Actor and Crew Release Form (NON-Student under 18)
This form is required for use by students hiring actors or crew members, under 18, who are not DePaul students when they participate in a DePaul film. The producer and/or director of the film is responsible for obtaining the signed form and retaining it for one year after the completion of the film project.
Actor and Crew Release Form (Student)
This form is required for use by all students who are DePaul students when they participate in a DePaul film. The producer and/or director of the film is responsible for obtaining the signed form and retaining it for one year after the completion of the film project.
Affidavit of Support
This form is used by international students in conjunction with the I-20 application process.
Assistantship Recommendation
This form is used ONLY by faculty outside CDM to recommend a current CDM graduate student for a graduate assistantship.
FERPA Waiver Form, Office of Graduate Admission
This form is used by graduate applicants to CDM to grant access of their educational records to a third party.
Grade Challenge
This form is used by current students to resolve grade disputes.
Incomplete Request
This online form is used by current students to request an incomplete grade.
Independent Study
Registration for independent studies are initiated by faculty on the CDM Intranet (under the Faculty tab) once the scope of the course is agreed upon and require final approval from the Associate Dean.
Internships
This online form is used by current students to request an internship for credit.
Late Enrollment
This online form is used by students who experience extreme and extenuating circumstances which prevented them from meeting the university enrollment deadline. The form is to be used by students appealing for late enrollment into CDM courses only.
Location Use Agreement
This form is required for use by a crew member that is filming on a location other than DePaul property. The agreement states that their project is covered by DePaul insurance. If the owner is satisfied with this agreement, the student has the owner sign and an approved faculty/staff member signs as a DePaul representative. Copies are made and each of the people signing retains a copy. The producer and/or director of the film is responsible for obtaining the signed form and retaining it for one year after the completion of the film project.
Retroactive Withdrawal
This online form is used by current CDM students to appeal for an exception to the University policy regarding course withdrawal.
SAG Student Film Letter of Agreement
This form is used to hire an actor who is a member of the Screen Actors Guild (SAG).
Transcript Return Request
This form is used by international students to request original academic document return.
Transfer Credit Request
This form is used by current CDM students to request evaluation and approval of transfer credit. Requests that exceed the allowable number of applied transfer credits (see policy here) are rare. Such request must be submitted online via MyCDM.
Transfer Release
This form is used by international graduate transfer students who wish to transfer their visa.
University/CDM Policy appeal
This online form is used by CDM students who have extenuating circumstances that preven them from adhering to a general University/CDM policy.
