Below are common forms current students might need over the course of their academic career.

Actor and Crew Release Form (over 18)
This form is used by actors or crew members, over 18, who are not DePaul students when they participate in a DePaul film.  The producer and/or director of the film is responsible for obtaining the signed form and retaining it for one year after the completion of the film project.

Actor and Crew Release Form (under 18)
This form is used by actors or crew members, under 18, who are not DePaul students when they participate in a DePaul film. The producer and/or director of the film is responsible for obtaining the signed form and retaining it for one year after the completion of the film project.

Affidavit of Support
This form is used by international students in conjunction with the I-20 application process.

Alumni Externship Registration
This form is used by alumni to create an externship profile for students.

Assistantship Recommendation
This form is used by faculty outside CDM to recommend a current graduate student for a graduate assistantship.

Combined Degree Registration
This form is used by students in the combined degree program to register for graduate courses while still enrolled as an undergraduate student.

DCC Peer Mentor Application
This form is used by current undergraduate digital cinema students to apply for a mentor position.

DCC Peer Mentor Request
This form is used by current undergraduate digital cinema students who wish to request a mentor.

Grade Challenge
This form is used by current students to resolve grade disputes.

Incomplete Request
This form is used by current students to request an incomplete grade.

Independent Study
This form is used by current students to request an independent study course.

Internships
This form is used by current students to request an internship for credit.

Retroactive Withdrawal
This form is used by current students to request a late withdrawal from a class.

SAG Student Film Letter of Agreement
This form is used to hire an actor who is a member of the Screen Actors Guild (SAG). 

SoC Peer Mentor Application
This form is used by current undergraduate students to apply for peer mentoring positions.

SoC Peer Mentor Request
This form is used by current undergraduate students who wish to request a mentor.

Student Ambassador Application
Apply to be one of a select group of volunteers to serve as student ambassadors who will assist in the recruitment and admission process at CDM.

Transcript Return Request
This form is used by international students to request original academic document return.

Transfer Credit Request
This form is used by current students to request evaluation of previously earned transfer credit.

Transfer Release
This form is used by international graduate transfer students who wish to transfer their visa.

U-Pass Waiver
This form is used by current graduate students who wish a waiver of the U-Pass fee.
