Commonly Used Forms

Below are common forms current students might need over the course of their academic career.

  1. Administrative Withdrawal
    This online form is used by current CDM students to appeal for an exception to the University policy regarding course withdrawal.
  2. Assistantship Recommendation
    This form is used ONLY by faculty outside CDM to recommend a current CDM graduate student for a graduate assistantship.
  3. Filmmaking Paperwork
    Actor and crew release forms, location agreements, project information forms, and all other forms and contracts related to student films are located at the link above or on SCA Central (D2L) under the “Info & Forms” tab.
  4. Grade Challenge
    This form is used by current students to resolve grade disputes.
  5. Incomplete Request
    This online form is used by current students to request an incomplete grade.
  6. Independent Study
    Registration for independent studies are initiated by faculty on the CDM Intranet (under the Faculty tab) once the scope of the course is agreed upon and require final approval from the Associate Dean.
  7. Internships
    This online form is used by current students to request an internship for credit.
  8. Late Enrollment
    This online form is used by students who experience extreme and extenuating circumstances which prevented them from meeting the university enrollment deadline. The form is to be used by students appealing for late enrollment into CDM courses only.

  10. Photo Release Form
    This form is required when photographing people whose image may be used in internal/external DePaul materials, including social media, advertisements, brochures, magazines, newsletters, and websites.
  11. Space Reservation Request
    Students, staff, and faculty can reserve rooms for meetings and events through 25 Live. To learn how to schedule an event and book a space, read the tutorial on CDM’s ServiceNow page.
  12. Transcript Return Request
    This form is used by international students to request original academic document return.
  13. Transfer Credit Request
    This form is used by current CDM students to request evaluation and approval of transfer credit. Requests that exceed the allowable number of applied transfer credits (see policy here) are rare. Such request must be submitted online via MyCDM.
  14. Transfer Release
    This form is used by international graduate transfer students who wish to transfer their visa.
  15. University/CDM Policy Appeal
    This online form is used by CDM students who have extenuating circumstances that prevent them from adhering to a general University/CDM policy.
